Principal's Weekly Message - May 30, 2017

Reported by on 5/30/17

Hello Spring Brook Parents,

Spring Brook Health Office Information

If your student currently has medications and/or medical supplies in the Spring Brook Health office. PLEASE READ:
Per district protocol, all medications are to be picked up by parents on the last day of school, Friday June 2nd. Last week information was sent home with your student regarding this information.
If you are unable to pick up your student’s medications on that date, Please contact the school health office to make arrangements. On June 6, all medications and /or supplies, which have been left at school will be disposed of.
We apologize for any inconvenience this causes and appreciate your efforts in keeping the students safe.

For your convenience, new district medication forms were included for next year. These are also part of the registration packet on the district website.
If your student will require medications to be kept in the Health Office during the school year,
Please carefully review the information forms sent home.
Most Medical offices will not fill out forms for physicals or orders for medications without a recent office visit. Please make every effort to schedule these before the beginning of school so that your students health needs are covered.
Thanks so much and have a wonderful summer,
Leslie Stubblefield RN BSN
